Disney Cruise Line Asking Passengers to Complete an Updated Travel History and Health Questionnaire

Last week, Disney Cruise Line began to send out emails to travel agents with guests booked on upcoming cruises with an updated Recent Travel History and Health Questionnaire to print, complete and bring to the port. This questionnaire is essentially the same as the one handed out last October following an Ebola incident on another cruise line with one difference. The previous questionnaire focused on travel to Guinea, Liberia or Sierra Leone in West Africa and having contact with a confirmed Ebola patient withing 60 days of the start of the cruise. The current questionnaire’s travel and contact window has dropped to 42 days.

The reason for the questionnaire revision appears to be a result of a May 9, 2015 travel notice update from the CDC. Center for Disease Control and Prevention (CDC) revised their Level 3 travel notice urging all US residents avoid nonessential travel to Guinea and Sierra Leone because of unprecedented outbreaks of Ebola in those countries. CDC recommends that travelers to these countries protect themselves by avoiding contact with the blood and body fluids of people who are sick, because of the possibility they may be sick with Ebola.

Additionally, the CDC no longer recommends US residents avoid nonessential travel to Liberia but does recommend travelers practice enhanced precautions when traveling to the country. CDC has Liberia as a Level 2 travel notice.

In the email, Disney Cruise Line stated that they do not have any Crew Members from these countries onboard our ships. In addition, DCL is actively screening all new and returning Crew regarding their recent travel history and close contacts prior to boarding. In an abundance of caution, DCL has decided to extend this screening to their Guests.

If you or a member of your stateroom party have traveled to the West African nations of Guinea, Liberia, or Sierra Leone or have been in direct physical contact (having cared for, lived with, or had direct contact) with a confirmed or suspected Ebola case within 42 days prior to the start of your cruise vacation, please contact us at 1-866-325-2112 or 407-566-3510.

Disney Cruise Line is asking guests to print and complete the Travel History and Health Questionnaire (PDF) for their stateroom party and bring it with you to check in for your cruise. Only one form is required per stateroom.

    Thanks Scott. It’s good to know these things in advance, not that I am discriminating. Ebola is a very serious disease. I just hope those traveling aboard DCL is honest on their questionnaire form.

      People would be a lot more honest on this questionnaire as well as the general health form if the cruise line would provide refunds in the event of denied boarding situation.

    I got mine yesterday (Memorial Day), kind of ironic isn’t it. 4-days to go before we are on our way to Vancouver!!!
